Confirmation Sponsor Eligibility Letter Form

This form is to be used for all St. Andrew parishioners who have been asked to be a Confirmation sponsor and need to provide a Letter of Eligibility from the parish.

You must be an active registered member of St. Andrew for at least six (6) months before calling to set an appointment with a priest.
